A 45‑year‑old male was referred with occludable angles in the right eye and open angles in the left eye. Visual acuity, intraocular pressure, and optic disc evaluation were normal in both eyes. Gonioscopy revealed that the right eye had 2 quadrants of open angle, with the remaining angles closed due to peripheral anterior synechiae, while the left eye showed an open angle.
Slit‑lamp examination revealed peripheral iris atrophic changes in the right eye and a persistent pupillary membrane in the left eye. A diagnosis of Axenfeld–Rieger syndrome with secondary angle closure in the right eye was made, and YAG peripheral iridotomy was advised.
