Image Summary
A 48-year-old woman was referred for ocular examination following a history of blunt trauma to the left eye after being kicked by a cow. Visual acuity was 6/6 in both eyes despite severe periorbital injury. Examination revealed marked periorbital ecchymosis, eyelid edema and localised subconjunctival haemorrhage. The cornea was clear with no significant hyphema or hypopyon. The case demonstrates that preserved visual acuity does not exclude occult ocular injury.
